If you have a child that would like to participate or know a parent who may have an interest in our program please download and complete the Participant Application and Medical Form. The link for those applications can be found below.
NOTE: Right click on the file and depending on the browser choose "Save Link As... (Firefox)" or "Save Target As... (Internet Explorer)" and then save to your computer. Proceed to open it from there.
This Participant Application is for both Football and Cheer. Please complete both applications and drop them off along with a certified copy of the child's birth certificate at one of our registration days. A list of registration days and sign-up information can be found here along with our address:
